I, the undersigned, acknowledge that paragliding is an adventure activity that involves inherent risks including but not limited to: injury, disability, and death. I voluntarily assume all risks associated with participating in paragliding activities organized by Oslob Cebu Paragliding Development (OCPD).
I release, waive, discharge, and hold harmless OCPD, its officers, employees, agents, and representatives from any and all liability, claims, demands, or causes of action that may arise from my participation in paragliding activities.
I confirm that I am in good physical health and have no medical conditions that would prevent me from safely participating in paragliding. I have disclosed any relevant medical conditions to the pilot and staff.
I agree to follow all safety instructions given by the pilot and staff. I understand that the decision to fly is ultimately at the discretion of the pilot based on weather and safety conditions.
